Role of Clinical Pharmacist in Chronic Disease Management

Abstract

Chronic diseases such as diabetes mellitus, hypertension, cardiovascular diseases, asthma, chronic obstructive pulmonary disease (COPD), and chronic kidney disease are among the leading causes of morbidity and mortality worldwide. The increasing prevalence of these conditions hascreated a significant burden on healthcare systems. Clinical pharmacists have emerged as essential members of multidisciplinary healthcare teams, contributing to improved patient outcomes through medication management, patient counseling, disease monitoring, and healthcare education. This review aims to evaluate the role of clinical pharmacists in chronic disease management and highlight their impact on medication adherence, disease control, patient safety, and healthcare costs. Evidence from various studies indicates that clinical pharmacist interventions significantly improve therapeutic outcomes, reduce medication-related problems, and enhance patients' quality of life. The integration of clinical pharmacists into chronic disease management programs is therefore crucial for achieving optimal healthcare outcomes.
